Background: BK virus nephropathy has an increasing role in renal transplant dysfunction, since new, highly potent immunosuppressive drugs have been introduced into therapy following renal transplantation. Diagnosis of acute impairment of renal transplant function is complicated by difficulty in differentiating BK virus nephropathy from acute rejection. Patients and methods: We retrospectively described the findings and therapeutic approaches of 6 consecutive patients with BK virus nephropathy in our transplantation center (75 – 80 transplantations/ year). BK virus nephropathy was classified according to Drachenberg et al. [2004]. Results: We observed an incidence rate of < 1% for BK nephropathy in our center. Four patients had a pattern B whereas 2 patients revealed a pattern C of BK virus nephropathy. Focal C4d-positive staining of peritubular capillaries were found in 2 of the 6 cases. For earlier detection of BK nephropathy, a diagnostic algorithm for each patient after renal transplantation was established. Urine was continuously monitored by cytology for decoy cells and PCR for BK virus DNA. If PCR was also positive for the BK virus in plasma, biopsy of the renal allograft was performed. Thereby diagnosis could be confirmed sooner. For treatment of BK nephropathy in our center, we reduced immunosuppressive agents and initiated a virustatic treatment with cidofovir in the first 3 cases. However, results were not satisfactory and two allografts were lost. We then reconsidered our therapeutic approach and switched the immunosuppressive treatment to leflunomide with consistent lowdose steroids. We use therapeutic drug monitoring for leflunomide and aim at a target level of 40 – 100 mg/ml. We lost no allograft with BK nephropathy since using this therapeutic approach. Conclusion: In our center, leflunomide therapy, but not cidofovir, was effective in patients with BK virus nephropathy of the renal allograft.Correspondence to:

Bakterielle Infektionen stellen nach wie vor für Patienten nach Nierentransplantation (NTx) wesentliche Komplikationen dar. Ziel der vorliegenden retrospektiven Multivarianzanalyse war es, die Risikofaktoren für Harnwegsinfektionen (HWI) zu analysieren sowie klinische und laborchemische Einflussgrößen bei nierentransplantierten Patienten eines Zentrums zu identifizieren. Untersucht wurden 329 Patienten, denen von 1990 – 2004 eine allogene Spenderniere transplantiert wurde. Die Beobachtungszeit betrug 3 Jahre. 195 Patienten erkrankten in den ersten 3 Jahren nach NTx an mindestens einer HWI (59,3%), im Mittel 43,5 ± 42,6 Tage nach NTx. Der Anteil einer asymptomatischen Bakteriurie (asB) betrug 89,7%. Dominierende Erreger waren E. coli und Enterokokken. Anhand der Multivarianzanalyse wurde festgestellt, dass Frauen und Patienten mit einem Diabetes mellitus oder einer HWI prae transplantationem besonders gefährdet sind, an einer HWI nach NTx zu erkranken. Kardiovaskuläre Vorerkrankungen, arterielle Hypertonie und die immunsuppressive Therapie standen nicht in signifikantem Zusammenhang mit Infektionen des Harntrakts. Patienten mit einer 3-monatigen Prophylaxe mit Cotrimoxazol (160/800 mg TMP/SMZ) wiesen im Vergleich zu Patienten ohne antibiotische Prophylaxe keine niedrigere Inzidenz auf.Correspondence to:

Lebendnierenspende – eine besondere Kasuistik?

Die Nierenlebendtransplantation hat einen festen Platz in der Behandlung terminal niereninsuffizienter Patienten erhalten, weil durch sie die Lebenszeit und -qualität des Empfängers wesentlich verbessert wird und das Risiko für den Spender vertretbar ist. Deshalb sollte die Möglichkeit dieses Organersatzes von den Nephrologen aktiver angesprochen werden. Unter den Kontraindikationen bei der Auswahl des Lebendspenders wird das Alter nur am Rande thematisiert. Die hier dargestellte Kasuistik einer 2. Nierenlebendtransplantation eines 55-jährigen, wieder dialysepflichtigen Patienten durch seinen hoch motivierten 83 Jahre alten Vater – beiden geht es 16 Jahre danach recht gut – soll darauf hinweisen, dass das kalendarische Alter des Spenders angehoben werden kann, wenn unter strengen Kautelen das biologische Alter sowie die Chancen und das Risiko bewertet werden.Correspondence to:
