Background: Hemodialysis patients very often suffer from malnutrition with net loss of body muscle and fat stores. Since protein-calorie malnutrition has been considered to be highly associated with increased morbidity and mortality, it is important that the nutritional status is evaluated accurately and that long-term evaluation of the nutritional status is performed to obtain good outcome. Patients and methods: Forty-six patients (28 men and 18 women) undergoing maintenance hemodialysis were studied. Nutritional status of the patients was examined by dual-energy X-ray absorptiometry (DXA) and anthropometry measurements after a hemodialysis session and 4 years after the initial measurement. Results: Percent fat, the ratio of body fat (BF) to weight, determined by DXA correlated positively with body mass index (BMI) calculated from weight and height. There was also a significant positive correlation between lean body mass (LBM) determined by DXA and arm muscle circumference (AMC) calculated using the anthropometric method. In the study period of 4 years, LBM and bone mineral content (BMC) observed at the end point were significantly lower than those at the start. In contrast, BF and %fat at the end point were significantly higher as compared with those at the initial measurement. Conclusion: These results indicate that DXA is a reliable method for body composition analysis in chronic hemodialysis patients. Because the detection of early alterations in body composition may provide an early indication of the development of malnutrition, serial evaluation of body composition using DXA should be valid for assessment of the nutritional status.

We describe the case of a young man with pseudohypoparathyroidism Type 1b – a rare genetic disorder characterized by end-organ resistance to parathormone (PTH) – and vitamin D intoxication sequelae due to inappropriate and poorly monitored calcitriol treatment in his adolescence, who could no longer be successfully treated by standard vitamin D treatment alone. Off-label administration of cinacalcet HCl, a calcimimetic approved for the treatment of secondary hyperparathyroidism, together with the vitamin D analog dihydrotachysterol, however, proved successful in controlling parathormone (PAH), bone-specific alkaline phosphatase (BAP), serum calcium, and phosphate levels.Correspondence to:

Objective: Diuretic therapy frequently induces undesirable biochemical changes and side effects. We compared metabolic effects of a low-dose diuretic (D) given in combination with an angiotensin II receptor antagonist, losartan (L), with those resulting from a diuretic given in combination with a calcium channel blocker, slow-release nifedipine (N). Material and methods: Thirty-seven elderly patients with mild to moderate hypertension (mean age: 71 ± 3 years) were treated with either L+D (n = 18) or N+D (n = 19) for 1 year. Diuretic therapy included low-dose trichlormethiazide or low-dose furosemide in numbers of patients that were similar between L+D and N+D groups. Blood pressure, serum electrolytes, uric acid, blood glucose, renal function and lipid parameters were measured at baseline, 6 months and 1 year. Results: Effective blood pressure control was observed in both groups at 6 months, and with further improvement at 1 year. Serum potassium was significantly decreased from baseline at 6 months (p < 0.01) and 1 year (p < 0.01) in the N+D group, but not in the L+D group. Serum uric acid was significantly increased from baseline at 6 months (p < 0.01) and 1 year (p < 0.01) in the N+D group, but had minimally decreased at 1 year in the L+D group (p < 0.1). Blood glucose, renal function and lipid parameters did not change in either group. Conclusion: The combination of losartan and low-dose diuretics effectively treated hypertension in elderly patients while minimizing the metabolic consequences of diuretic therapy. B-type natriuretic peptide and severe heart failure at baseline predict overall mortality in incident dialysis patients

Aims: The B-type natriuretic peptide (BNP) has become increasingly important as a diagnostic and prognostic method for cardiovascular disease or death. To our knowledge no prospective studies exist to evaluate the value of baseline BNP and baseline heart failure as predictors of overall death in incident rather than prevalent hemodialysis patients with end-stage renal disease (ESRD). Methods: 255 ESRD patients were included in our observational study with a median observation period of 1.11 years. A Kaplan-Meier survival curve was stratified by BNP concentration (< 340 pg/ml and ≥ 340 pg/ml) to estimate the impact on the overall mortality rate. Univariate and multiple Cox regression models were fitted for a variety of covariables including severe heart failure (graded according to the New York Heart Association) to evaluate the independent predictors of death. Association between BNP and four explanatory variables was described in a multiple linear regression model. Results: Survival analysis demonstrated a significantly higher mortality rate in patients with higher BNP values at baseline. The independent predictive value of high BNP concentration at baseline could be statistically confirmed by multiple Cox regression analysis. However, when including the covariates hemoglobin and severe heart failure, significantly associated with BNP, in the same model, severe heart failure rather than BNP becomes a significant predictor of overall death. Conclusions: A higher BNP level at baseline may be confirmed as an independent predictor of death in the incident dialysis population. However, severe heart failure may affect the impact of BNP on the overall survival rate and thus be a stronger predictor of death than BNP.Correspondence to:

Die folgende Kasuistik beschreibt den Fall eines jungen Mannes mit Pseudohypoparathyreoidismus Typ 1b – einer seltenen genetischen Erkrankung, die durch eine Endorgan-Resistenz gegen Parathormon (PTH) charakterisiert ist. Da die Erkrankung mit einer Standard-Vitamin-D-Therapie alleine nicht mehr behandelbar war, erfolgte eine unzureichend überwachte Vitamin-D-Therapie, in deren Folge es zu einer Vitamin-D-Intoxikation kam. Durch eine Off-Label-Therapie mit Cinacalcet-HCl, einem Kalzimimetikum, das für die Behandlung des sekundären Hyperparathyreoidismus zugelassen ist, und gleichzeitiger Gabe des Vitamin-D-Analogs Dihydrotachysterol konnten jedoch die Konzentrationen von Parathormon (PTH), knochenspezifischer alkalischer Phosphatase (BAP), Serum-Kalzium und Phosphat erfolgreich reguliert werden.Correspondence to:

Zielsetzung: Das B-Typ-Natriuretische Peptid (BNP) spielt eine große Rolle bei der Diagnose und Prognose von kardiovaskulären Erkrankungen oder kardiovaskulärem Tod. Unseres Wissens ist bislang keine prospektive Studie durchgeführt worden, in der der Aussagewert des BNP und der Herzinsuffizienz zu Studienbeginn als Prognosefaktoren für die Gesamtsterblichkeit bei inzidenten (nicht bei prävalenten) Hämodialysepatienten mit terminaler Niereninsuffizienz untersucht worden wäre. Methoden: In die Beobachtungsstudie wurden 255 Patienten mit terminaler Niereninsuffizienz eingeschlossen. Der mediane Beobachtungszeitraum betrug 1,11 Jahre. Um den Einfluss auf die Gesamtmortalitätsrate zu analysieren, wurde eine Überlebenskurve nach Kaplan-Meier nach BNP-Konzentrationen (< 340 pg/ml und ≥ 340 pg/ml) stratifiziert. Univariate und multiple Modelle der Cox-Regression wurden für verschiedene Kovariablen, wie zum Beispiel schwere Herzinsuffizienz (gemäß der Einteilung der New York Heart Association), adjustiert, um die unabhängigen Prognosefaktoren für Mortalität zu evaluieren. Der Zusammenhang zwischen dem BNP und vier erklärenden Variablen wurde in einem multiplen linearen Regressionsmodell beschrieben. Ergebnisse: Die Überlebensanalyse zeigte eine signifikant höhere Mortalitätsrate bei den Patienten mit höheren BNP-Werten zu Dialysebeginn. Der unabhängige Prognosewert einer hohen BNP-Konzentration zu Dialysebeginn konnte statistisch durch eine multiple Cox-Regressionsanalyse bestätigt werden. Schließt man in dasselbe Modell jedoch die Kovariablen Hämoglobin und schwere Herzinsuffizienz, die signifikant mit dem BNP assoziiert sind, mit ein, so liefert die schwere Herzinsuffizienz signifikantere Prognosewerte für die Gesamtsterblichkeit als das BNP. Schlussfolgerungen: Es konnte gezeigt werden, dass ein höherer BNP-Spiegel zu Dialysebeginn bei inzidenten Dialysepatienten einen unabhängigen Prognosefaktor für den Tod darstellt. Allerdings kann eine schwere Herzinsuffizienz die Bedeutung des BNP auf die Gesamtüberlebensrate beeinflussen und ist somit ein aussagekräftigerer Prognosefaktor für den Tod als das BNP.Correspondence to:

Trotz vielerlei Anstrengungen starten viele Patienten die Dialyse akut und in aller Regel mit einem zentralvenösen Katheter, der das Infektions- und Sterberisiko in den ersten Monaten deutlich erhöht. In den letzten Jahren hat sich die Peritonealdialyse (PD) auch zum akuten Dialysebeginn etabliert, wird aber wegen mangelnder infrastruktureller Voraussetzungen wenig angewandt. Die PD ist ein schonendes, sicheres, komplikationsarmes und effektives Verfahren, das sich mittlerweile weltweit in vielen Zentren bewährt hat und sich besonders gut zur Dialyseeinleitung bei älteren und herz-nierenkranken Patienten eignet, die wenig Reserve haben, dem Hämodialysestress der ersten Monate etwas entgegenzusetzen. Ein Ausbau der infrastrukturellen Voraussetzungen könnte das Überleben dieser schwerkranken Patienten verbessern. In diesem Beitrag wird die PD-Therapie als initiale Dialysemethode, der akute und ungeplante PD-Beginn mit infrastrukturellen und logistischen Anforderungen dargestellt.Correspondence to:

Akute respiratorische Infektionen im Kindesalter führen häufig zur Vorstellung beim Kinderarzt und zur Verschreibung von Antibiotika. Veränderte epidemiologische Situationen, zunehmende Resistenzentwicklung von Bakterien gegenüber gängigen Antibiotika und Neuerungen im Bereich der molekularen Diagnostik erfordern, dass die Evidenz für Diagnosestellung und Behandlung von ARIs im Kindesalter immer wieder neu bewertet wird. Diese Übersichtsarbeit will anhand einer umfassenden Literaturrecherche den aktuellen Stand der Wissenschaft zur Diagnostik und antibiotischen Therapie von ARI im Kindesalter ab 2020 bis dato zusammenfassen, wobei auch globale Aspekte berücksichtigt werden sollen.Correspondence to:

Einsatz von Biologika bei CRSwNP – Aktuelle Registerdaten im Überblick

Monoklonale Antikörper ergänzen seit mehr als 5 Jahren die Therapie der schweren, unkontrollierten chronischen Rhinosinusitis mit Nasenpolypen (CRSwNP). Ihr Sicherheitsprofil und ihre Wirksamkeit konnten in großen randomisierten, placebokontrollierten Zulassungsstudien bestätigt werden. Darüber hinaus führen Real-World-Studien und insbesondere Registerstudien zu weiteren Erkenntnissen über den Langzeitnutzen und die Therapiesicherheit sowie die Charakterisierung der Patienten im klinischen Alltag. Diese Informationen sind essentiell, um den betroffenen Patientinnen und Patienten die bestmögliche Therapie ihrer chronischen Erkrankung zu gewährleisten. Eine klare Evidenz für die Wirksamkeit und Sicherheit von Dupilumab in der Therapie der CRSwNP kann durch aktuelle Registerstudien belegt werden – zum Therapieansprechen von Mepolizumab und/oder Omalizumab stehen diese Real-World Studien derzeit noch aus und sollten angestrebt werden.Correspondence to:
