A female dialysis patient with a consistently high serum calcium phosphate product presented with large necrotic skin lesions with ulcers. The clinical course was highly suggestive of calciphylaxis. Parathyroidectomy was followed by the healing of the lesions. New skin lesions appeared following relapse of hyperparathyroidism. Her clinical records included a long past of hypertension, which was the cause of her renal failure. She had a limited walking range and previously had presented bilateral ulcers of vascular origin. This case presents a type of lesion which bears a serious prognosis in dialysis patients. The clinical context and the presentation of the lesions are compatible with multiple etiology: vascular lesions and calciphylaxis. The documented longitudinal follow-up illustrates the importance of treating the different factors known to participate in the appearance of skin lesions in dialysis patients. Particularly, it stresses the benefit of performing parathyroidectomy, even if the parathyroid hormone level is not in the range normally accepted as requiring surgical removal of parathyroid glands.

Fewer dose changes with once-monthly C.E.R.A. in patients with chronic kidney disease

Background: Frequent dosing and requirements for dose adjustments of erythropoiesis-stimulating agents (ESAs) create significant burdens for healthcare providers and have been associated with hemoglobin (Hb) cycling, hampering maintenance of target Hb levels. We compared the frequency of dose changes in dialysis patients who received methoxy polyethylene glycolepoetin beta; (a continuous erythropoietin receptor activator (C.E.R.A.)) or a shorter-acting ESA. Methods: Data were analyzed from three Phase III maintenance trials, using almost identical protocols, in dialysis patients treated with C.E.R.A. every 2 weeks (q2w) or every 4 weeks (q4w) or a comparator ESA (epoetin or darbepoetin alpha; at their previous dose/administration interval). Dosage was adjusted to maintain Hb ± 1 g/dl of baseline and 10 – 13.5 g/dl during titration (28 weeks) and evaluation (8 weeks), and 11 – 13 g/dl during follow-up (16 weeks). Results: Data were analyzed from 564 patients treated with C.E.R.A. q2w, 410 with C.E.R.A. q4w and 572 with comparator ESA at their usual dosing interval. Significantly fewer dose changes were needed in patients receiving C.E.R.A. q2w (p < 0.05) or C.E.R.A. q4w (p < 0.001) than in patients treated with comparator ESAs. Conclusion: This retrospective analysis suggests that C.E.R.A. q4w maintains Hb levels in dialysis patients and requires fewer dose changes compared with other ESAs.Correspondence to:

Geringere Anzahl von Dosisveränderungen bei einmal monatlicher Verabreichung von C.E.R.A. bei Patienten mit chronischer Niereninsuffizienz

Hintergrund: Kurze Dosierungsintervalle und die Notwendigkeit von Dosisanpassungen von Erythropoese stimulierenden Wirkstoffen (ESA) stellen eine große Belastung für die Gesundheitssysteme dar und sind mit schwankenden Hämoglobin (Hb)-Spiegeln (sog. Hämoglobin-Cycling) assoziiert, was die Aufrechterhaltung der angestrebten Hb- Spiegel erschwert. In der vorliegenden Untersuchung wurde die Häufigkeit von Dosisveränderungen bei Dialysepatienten, die Methoxypolyethylenglycol-Epoetin-β (einen kontinuierlichen Erythropoetin-Rezeptor- Aktivator (C.E.R.A.)) bzw. einen kürzer wirksamen ESA erhielten, verglichen. Methoden: Analysiert wurden die Daten aus drei Phase III Erhaltungsstudien mit beinahe identischen Studienprotokollen. Die Studien waren an Dialysepatienten durchgeführt worden, die entweder alle 2 Wochen (q2w) oder alle 4 Wochen (q4w) C.E.R.A. beziehungsweise einen Vergleichs-ESA (Epoetin oder Darbepoetin-α Dosierung/Interval wurden wie vor Beginn der Studie weitergeführt) erhielten. Die Dosierung wurde so angepasst, dass ein Hb-Wert ±1 g/dl im Vergleich zum Wert bei Studienbeginn und von 10 –13,5 g/dl wärend der Titrations- (28 Wochen) und Evaluationsphase (8 Wochen) sowie von 11 –13 g/dl wärend des Followup (16 Wochen) erreicht wurde. Ergebnisse: Folgende Daten wurden analysiert: 564 Patienten mit C.E.R.A. q2w, 410 Patienten mit C.E.R.A. q4w, 572 Patienten mit Vergleichs- ESA im für sie üblichen Dosierungsintervall. Bei den Patienten, die C.E.R.A. q2w (p < 0,05) bzw. C.E.R.A. q4w (p < 0,001) erhielten, waren signifikant weniger Dosisanpassungen erforderlich als bei den Patienten mit Vergleichs-ESA. Schlussfolgerung: Diese retrospektive Analyse lässt darauf schließen, dass C.E.R.A. q4w die Hb-Spiegel bei Dialysepatienten stabil hält und weniger Dosisveränderungen erfordert als andere ESA.Correspondence to:
