Abstrakt

Introduction: Obstructive sleep-apneasyndrome (OSAS) and gastroesophageal reflux disease (GERD) both have a high prevalence. Excessive daytime sleepiness (EDS) is a key symptom as in apnea as in reflux. We evaluated the coincidence in OSAS and GERD and whether reflux-specific disturbances of micro-sleep architecture could be detected. Patients and methods: 11 consecutive patients with EDS with preceding pathological cardio-respiratory monitoring. Full polysomnography with timesynchronic longterm pH-measurement with laryngopharyngeal reflux diagnosis (LPR). ESS > 8, PST and Quatember-Maly pathologic. Results: In all cases, the LPR was pathologic with a high arousal index mainly in the form of CAP arousals. These are accumulated in sleep phases with low AHI. HF and PTT variability were detected in same expression as in apnoe-induced arousals as in pH-dependant arousals. The LPR probe was well tolerated by all patients. Conclusion: OSAS und GERD are apparently often coexisting diseases. Both are characterized by marked daytime sleepiness. Reflux events lead to sleep fragmentation with disruption of micro-sleep microstructure. Even in asymptomatic patients without specific reflux symptoms the possibility of “silent” reflux should be considered in patients with increased daytime sleepiness and striking disturbances of sleep micro-structure are documented.
