Abstrakt

Psychosocial care has become a crucial cornerstone of interdisciplinary care at specialized out-patient centers for young CF patients. Continued counseling and support from the very start of treatment is of great importance. Major areas of work are the following: medical counseling about the diagnosis and coping with the diagnosis, developing the structure of the patient’s daily life in alignment with the stress-bearing capacity of all family members; considering the specific family dynamics of living with a chronically ill family member, accompanying the person in transitional situations and crises; integration into social institutions such as children’s day-care centers, school, training institution or workplace; social and legal issues; support of adolescent and young adult patients in developing their autonomy and switching to the out-patient department for adults; intensive cooperation and case-related work with all professional groups of the CF team with the aim of developing a common treatment concept adjusted to the patient and his/ her family.
