Abstrakt
In Austria training in Pneumology is performed mainly in an inpatient setting. Special centers for rehabilitation (formerly used to treat tuberculosis) perform rehabilitation in a multidisciplinary setting. Training consists of endurance and strength training for the upper and lower extremities and inspiratory muscle training for COPD-patients in Gold stage II and higher. The training in an out patient setting is only possible in a few centers and has to be paid by the patient himself. In 2010 the PVA (Pensionsversicherung = pension fund for white collar workers and blue collar workers) will pay training therapy for COPD-patients in 16 centers for out-patient rehabilitation. We present a prospective study in an outpatient setting by H. Zwick. 83 COPD patients (phase 2, 3 and 4) performed endurance and strength training two times a week for 1 year. As a result they found a significant improvement of the SGRQ (baseline 37,2 and 26,5 after 1 year). Significant improvements of strength in the upper and lower limb. Also a significant improvement of the inspiratory muscle strength (baseline 81 mbar and 108,8 mbar after 1 year) and a significant reduction of hospital stays (at baseline 2,5/year and 0,34 after 12 months) and days in hospital (at baseline 27,3 and 3,5 after 12 months).
