Abstrakt

Autoimmune systemic diseases and glomerular diseases represent a significant cause of progressive renal impairment. The early identification of active glomerular inflammation is crucial for prognosis, as a delayed diagnosis and consequently a delayed start of therapy can lead to irreversible glomerular and tubulointerstitial damage. In clinical practice, urine diagnostics play a central role. In particular, manual urine sediment analysis enables early differentiation between glomerular and non-glomerular hematuria and also provides important clues regarding inflammatory activity in the kidney. This article summarizes clinical warning signs as well as typical findings in manual urine sediment analysis, which together enable the early diagnosis of glomerular diseases. The focus is on hematuria as the leading symptom, both in otherwise healthy, asymptomatic individuals and in patients with suspected (rapidly progressive) glomerulonephritis, which, as a nephrological emergency, requires immediate action.
