Prevention of vascular kidney disease: Monitoring and optimizing of vascular risk factors

Abstrakt
The Kidney is a highly vascular organ.
Micro- and macrovascular kidney damage, caused by risk factors of the metabolic syndrome mainly as arterial hypertension and diabetes, are the main causes for the need of renal replacement therapy in Germany. Monitoring and optimized treatment of the vascular risk factors is indicated on account of three significant reasons:
1) Primary Prevention: a cluster of risk factors with arterial hypertension, diabetes and other disorders of the glucose metabolism, hyper- and dyslipidemia, sedentary lifestyle, hyperalimentation resulting in overweight and obesity, hyperuricemia, and smoking is the main pathophysiological causation of vascular kidney disease. Prevention and optimal control of these factors is the major goal to impede vascular CKD.
2) Secondary Prevention: Since progression of CKD is driven essentially by arterial hypertension and diabetes, the optimal control of these vascular risk factors is the standard treatment for decelerating CKD worsening. For an improved outcome, therapeutic substances with organoprotective effects beyond blood pressure and glucose lowering should be applied (such as blocker of the RAAS, SGLT2-inhibitors, GLP1-Receptoragnosits).
3) Tertiary Prevention: Cardiovascular disease and CKD share several vascular risk factors, and CKD itself is an independent risk factor for cardiovascular morbidity and mortality. Risk factor treatment according to guideline recommendations, i. e. control of blood pressure, glucose metabolism, lipids, normalization of body weight, well-balanced and healthy nutrition as well as personalized boosted physical activity might result in improved renal and cardiovascular outcomes.
