Abstrakt

A structured aftercare program following pulmonary embolism may help to detect postembolic functional impairment and should help to manage anticoagulation in order to prevent recurrency and bleeding as well. Risk assessment for recurrency and treatment associated bleeding are challenging, but important for choosing the right treatment strategy. Risc calculators may be helpful to determine the length of anticoagulation. At diagnosis of venous thromboembolism acute pulmonary embolism, an acute on chronic event or a primarily chronic thromboembolic disease should be discriminated. A residual functional impairment following pulmonary embolism has been described and might be the consequence of a lack of training and a posttraumatic stress disorder, interfering comorbidities or chronic thromboembolic disease. Interfering comorbidities may yet been unknown at the time of diagnosis of PE or may be preexisting. Inflammatory and immunologically driven comorbidities and malignancies as well may cause pulmonary embolism and cause fatigue. Cardiovascular comorbidities have to be excluded as the cause of functional impairment. Diagnostic strategies to detect occult malignancies and even cardiovascular diseases should be rational and cost-effective. The history including the assessment of recommended prevention examinations should be carefully taken and a structured physical examination has to be performed. Laboratory test and ultrasound can provide additional information. In case of suspicious findings further diagnostic work-up should be recommended.
