Abstrakt

Chronic thromboembolic pulmonary hypertension (CTEPH) is a form of pulmonary hypertension (PH) that develops after incomplete resolution of thrombi following pulmonary embolism. It leads to increased pulmonary vascular resistance due to fibrothrombotic obstructions and associated microvasculopathy. Diagnosis includes, among other methods, ventilation–perfusion scintigraphy, CT and subtraction angiography, as well as right heart catheterization. Interdisciplinary diagnosis by a specialized team (MDT) is essential. Therapeutic options include pulmonary endarterectomy (PEA), balloon pulmonary angioplasty (BPA), and medical treatments (riociguat and treprostinil). Treatment decisions are made individually within the MDT. PEA is the treatment of choice for operable CTEPH and is associated with excellent long-term survival. In inoperable cases or in residual PH after PEA, BPA and medical therapy are effective. A multimodal therapeutic approach is increasingly being established. Reevaluation after completion of therapy is recommended. Thanks to specialized centers and modern treatment strategies, the 3-year survival rate has now increased to over 90%.
