Abstrakt

Acute pulmonary embolism (PE) can lead to obstructive shock due to an embolus-induced vascular occlusion of the pulmonary arterial circulation. In the setting of shock, echocardiography is particularly valuable for diagnostic assessment. This review article focuses on emergency therapy in cases of existing or impending hemodynamic instability. Key elements of reperfusion therapy, such as systemic thrombolysis and interventional procedures, must be carefully weighed against the patient’s risk of mortality. Especially in the area of interventional techniques, prospective studies comparing these approaches with standard therapy are eagerly awaited. Extracorporeal circulatory support (ECLS) has shown relatively high survival rates, even during resuscitation, and can serve as a bridging therapy in this clinical setting. A well-founded, ideally interdisciplinary decision-making process within a pulmonary embolism response team (PERT) forms the foundation for optimal patient care in cases of potentially life-threatening pulmonary embolism.
