Predictive risk factors and screening in Barrett’s esophagus – approaches to pharmacological prevention

Abstrakt

The incidence of esophageal adenocarcinoma (EAC) has been rising rapidly in the Western world for decades. At the same time, the 5-year survival rate for patients with EAC in Germany is less than 26% – one of the worst prognoses among all types of cancer. Barrett’s esophagus plays a central role as the only known precancerous condition and increases the risk of developing EAC by 30 to 125 times. Early detection of neoplastic changes and the correct management of high-risk patients are therefore essential components of preventive medicine. The following article highlights differentiated predictive risk factors for the development of EAC as well as new findings on risk stratification and diagnostics. An individualised approach focusing on predictive factors, selective screening and targeted chemoprophylaxis offers potential for reducing incidence and mortality.
