Histopathological Diagnosis of Barrett’s esophagus

Abstrakt

Barrett’s mucosa constitutes a metaplastic transformation of the distal esophageal epithelium and is associated with an increased risk of progression to dysplasia and, ultimately, esophageal adenocarcinoma. Although recent evidence suggests that this risk is substantially lower than previously estimated, the diagnosis of Barrett’s mucosa remains clinically significant. Molecular alterations implicated in carcinogenesis can, in some cases, be identified within the metaplastic epithelium prior to the development of invasive carcinoma. Diagnostic criteria for Barrett’s mucosa are not uniformly defined across international guidelines, and the histopathological assessment of dysplasia is inherently complex. Consequently, such evaluations should be performed or confirmed exclusively by experienced gastrointestinal pathologists. The histological classification of invasive carcinomas is critical for guiding clinical management and must comply with established diagnostic standards.
