Challenges in the histological confirmation of pulmonary nodules

Abstrakt

A precise and timely histological confirmation of a pulmonary nodule is essential for the prompt initiation of potentially curative therapy and may reduce the risk of locoregional progression and distant metastasis. In addition to the established CT-guided transthoracic biopsy, numerous minimally invasive bronchoscopic techniques are now available, whose diagnostic performance has markedly improved due to technological advances. In particular, the use of radial endobronchial ultrasound (r-EBUS), electromagnetic navigation systems, cone-beam CT, and robotic-assisted bronchoscopy has significantly increased diagnostic yield while maintaining low complication rates. Although CT-guided biopsy long demonstrated the highest diagnostic sensitivity, it is associated with a substantially higher rate of pneumothorax compared with endoscopic approaches. Modern navigational and robotic techniques increasingly enable the safe biopsy of very small (<1 cm) and peripherally located pulmonary lesions. Selection of the optimal diagnostic modality should take into account lesion characteristics, patient-specific risk factors, technical availability, and interdisciplinary decision-making.
