Radiological imaging in lung cancer screening – opportunities and challenges

Abstrakt

Low-dose computed tomography (LDCT) is the diagnostic core of lung cancer screening programmes and enables detection of early-stage disease, leading to a significant reduction in lung cancer mortality. Advances in hardware, dose optimisation, software-assisted reading and standardized systems such as Lung-RADS improve sensitivity, reproducibility and quality assurance. LDCT additionally provides information on cardiovascular and pulmonary comorbidities. However, limitations include false-positive and false-negative findings, cumulative radiation exposure, resource demands and ethical as well as methodological challenges. Structured quality management, collaboration with specialised centres and continuous evaluation – including innovative biomarkers and radiomics – are essential for sustainable programme success.
