Composition of ad hoc ethics consultations in acute and emergency clinical medicine and intensive care medicine

Abstrakt
Ethical decision-making in emergency situations poses particular challenges for the treating teams. Despite fundamental structural differences, emergency rooms and intensive care units have in common that complex ethical dilemmas must be solved under time pressure. In order to make the most comprehensive, well-founded and patient-centred decision possible, team reflection is necessary even in time-critical situations. To this purpose, ad hoc ethics consultations can be convened at short notice, even outside regular working hours. This article outlines suggestions for the staffing of an ad hoc advisory group. In addition to the specialist physician in charge of treatment in the respective care area, this should include the primary nurse, representatives of other disciplines involved in the treatment and, depending on availability, a palliative care physician, a member of the crisis intervention team (CIT) or the hospital chaplaincy, and the patient or his or her legal representative. Parallel to the implementation of ad hoc ethics committees, structures for training and further education in ethical decision-making should be implemented.
