Abstrakt

Ethical decision-making in acute and emergency clinical medicine takes places under special structural conditions that differ from both preclinical emergency medicine and intensive care medicine. Time pressure, diagnostic uncertainty, incomplete information about the patient’s wishes, and early therapeutic decisions characterize the decision-making environment of the central emergency room. This article describes the specific framework conditions and central ethical conflicts in this setting and highlights medical indication and patient wishes as the fundamental normative axes of medical decision-making. In addition, empirical findings on moral distress in acute and emergency medicine are discussed, along with their significance for decision-making quality and team culture. Finally, pragmatic, low-threshold structuring approaches are outlined that enable ethical orientation under acute conditions without hindering the clinical process. The aim is to highlight ethical decision-making in clinical acute and emergency medicine as an independent core competence and to offer practical guidance.
