Abstrakt

According to current data from the Robert Koch Institute (RKI), approximately two-thirds of men (67%) and more than half of women (53%) in Germany are classified as overweight (BMI > 25 kg/m2). The progression of the obesity epidemic and its associated comorbidities over recent decades is widely recognized as a multifactorial phenomenon. Contributing factors include genetic, epigenetic, and microbial determinants, as well as environmental influences such as excessive caloric intake, insufficient physical activity, and psychosocial stress. Whereas obesity of grade I can often be effectively managed through structured interventions involving dietary modification, physical activity, and behavioral therapy, evidence-based treatment strategies for grade II and grade III obesity are limited to two primary approaches: (1) participation in comprehensive multimodal weight reduction programs of 6 – 12 months duration, or (2) metabolic and bariatric surgery in cases where conservative measures prove unsuccessful. Endoscopic interventions represent an additional option for individuals with overweight or obesity who are either unsuitable candidates for, or unwilling to undergo, surgical procedures. Importantly, sustained success across all therapeutic modalities necessitates continuous, lifelong follow-up care to prevent weight regain and ensure long-term health outcomes.
