Food allergies and food intolerances in gastroenterological practice

Abstrakt
Food-related adverse reactions comprise a heterogeneous group of conditions including food allergies, non-allergic hypersensitivity reactions, carbohydrate malabsorption, and celiac disease. The prevalence of food allergies is increasing, particularly in industrialized countries, and may result in severe or potentially life-threatening reactions. Diagnosis requires a structured clinical history, allergological testing, and, when appropriate, oral food challenges with careful interpretation of clinical relevance. Management focuses on emergency medication, individualized elimination diets, and regular reassessment, while novel immunomodulatory approaches show promising results. Carbohydrate malabsorption, particularly lactose and fructose malabsorption, represents a common cause of gastrointestinal symptoms and is primarily diagnosed using hydrogen breath testing. Treatment is based on targeted dietary reduction rather than strict elimination. Celiac disease is a common autoimmune disorder of the small intestine with a highly variable clinical presentation; diagnosis relies on serological testing and intestinal biopsy under a gluten-containing diet. Lifelong adherence to a gluten-free diet remains the only established therapy.
