Abstrakt

Recently, the therapeutic options of metastatic non-small-cell lung cancer have been vastly enriched. As a new standard of first-line systemic therapy, the use of anti PD1/anti-PD-L1 checkpoint inhibitors with or without platinum-based chemotherapy has been established. However, the majority of patients will finally be confronted with disease progression. The timepoint of progression, the response to the previous therapy, the type and prevalence of molecular alterations as well as the number of progressing localizations is important information that influences the choice of the subsequent therapeutic approach, e.g., systemic versus locally ablative. As subsequent systemic therapy, most physicians will choose docetaxel with or without concomitant anti-angiogenic treatment. Still, this treatment was established when no checkpoint inhibitors were in clinical use, so data on efficacy of these regiments is sparse. Several approaches are being developed with the focus on enhancing the efficacy of activation of the patient’s immune system.
