Abstrakt

Talc poudrage and treatment of complicated parapneumonic effusion/empyema are therapeutic indications of medical thoracoscopy. The procedure is performed under local anesthesia, usually with one thoracic access and under analgosedation. The aim of talc poudrage is the pleurodesis to stop the recurrence of pleural effusion. The result is a significant improvement in the patient’s quality of life. This method is mainly used in malignant but in individual cases also in uncontrollable benign effusions when all possibilities of therapy of the underlying diseases have been exhausted. Aims of medical therapy of pleural infections are control of infection, re-expansion of the lung, and reduction of mortality. Medical thoracoscopy allows adhesiolysis, opening of chambers, and the evacuation of pus if thoracocentesis and saline rinse therapy with chest tube alone or with pleural fibrinolysis cannot be used efficiently. Efficiency rates are high for both indications. They are more than 90%. The procedure is efficient, gentle for the patient, and low in complications.
