Whole Body Vibration in hemodialysis patients – beneficial effects particularly in patients with impaired physical functions

Abstrakt

Objective: Structured endurance and strength training is beneficial in the therapy of musculoskeletal disturbances for patients on maintenance hemodialysis (MHD). Due to severe clinical complications and lack of motivation an active training cannot be performed by many patients. Therefore, a pilot study was conducted to determine the effects of Whole Body Vibration Training (WBVT) on parameters of physical performance, blood chemistry and tolerability.
Methods: In a prospective study 14 patients on MHD were treated 2-times per week for 12 weeks. 8 patients were treated before and 6 after hemodialysis. Primary endpoint was the physical performance, quantified by means of the Short Physical Performance Battery (SPPB) including balance test, usual gait speed and chair rise test (5-times). Secondary endpoints included 6-minute walk test, the timed up and go test, hand grip test, Bio-Impedance Analysis to measure body composition, blood chemical markers and physical activity parameters.
Results: As compared to pre-treatment levels, a significant improvement of SPPB values was detected after the training period (p = 0.035). Non-significant betterments were observed in 6-minute walk test, timed up and go test, jumping height, hand grip strength, skeletal muscle index and physical activity. Strongest improvements were observed in patients with particularly poor physical initial values. The blood investigation exhibited a decrease in the number of leukocytes and an insignificant reduction of high sensitive C-reactive protein (hs-CRP).The training was well tolerated if performed before dialysis. Training after hemodialysis resulted in 2 patients in a momentary decline of blood pressure due to the dialysis induced volume contraction.
Conclusion: According to our pilot study, WBVT appears to be a safe and feasible method to improve muscle deficits, particularly in dialysis patients with marked functional impairments. Parameters of systemic inflammation improved. These results should be confirmed in further studies including larger patient cohorts.
