What does inpatient rehabilitation achieve with regard to the rehabilitation goal of coping with pain in patients after hip or knee endoprosthesis implantation?

Abstrakt

Background: The improvement of pain management is a priority goal in rehabilitation after implantation of a hip or knee joint endoprosthesis. We investigated the changes in individual pain perception and the use of analgesics in inpatient musculoskeletal rehabilitation. Method: Across Germany, data on pain sensation and analgesic medication of patients after hip or knee replacement (N = 2,312/1,767) were collected in 44 inpatient rehabilitation facilities. A combination of numerical rating scale (NRS) and visual analogue scale (VAS) was used to measure pain. Data were collected at the beginning (T1) and at the end (T2) of rehabilitation (pre-post design). Results: Patients after hip joint replacement: At T1, 8.9% of patients with hip replacement had no pain, 27.7% had mild pain, 54.4% moderate pain, 8.4% severe pain, and 0.6% unbearable pain. At T2, the number of patients with hip replacement who were pain/free increased to 48.4% (42.3% had mild pain, 8.6% moderate pain, 0.6% severe pain, and 0.1% unbearable pain). Analgesic medication could also be reduced: no analgesics T1: 3.1%, T2: 24.3%, on-demand medication T1: 8.0%, T2: 36.2%, WHO stage I T1: 63.2%, T2: 30.1%, WHO stage II T1: 18.2%, T2: 6.3%, and WHO stage III T1: 7.5%, T2: 3.1%. Patients after knee joint replacement:
At T1 2.7% of patients with knee replacement had no pain, 17.2 patients had mild pain, 64.1% moderate pain, 14.6% severe pain, and 1.5% unbearable pain. At T2, the number of patients with knee replacement who were pain-free increased to 27.7% (53.3% patients had mild pain, 17.9% moderate pain, 0.9% severe pain, and 0.2% unbearable pain). Analgesic medication could also be reduced: no analgesics T1: 1.9%, T2: 11.1%, on-demand medication T1: 5.8%, T2: 28.7%, WHO stage I T1: 51.0%, T2: 40.2%, WHO stage II T1: 27.2%, T2: 12.6%, and WHO stage III T1: 14.1%, T2: 7.4%. Conclusion: In patients after hip or knee arthroplasty, the pain symptoms decreased after inpatient musculoskeletal rehabilitation. At the same time, analgesic medication was reduced.
