Abstrakt

The extrinsic-allergic alveolitis (EAA) is a rare complex pulmonary disease mediated by the immune system and caused by inhaling antigens. The most common antigens that cause EAA worldwide are proteins and bacteria. However, more and more common moulds are responsible for EAA, which are frequently observed in energetically renovated buildings with poor ventilation. As the EAA continues to progress in the event of continued exposure, it is often necessary to take expensive measures to prevent mould exposure. The diagnosis and differentiation against other interstitial lung diseases is difficult because there is no specific test available. Therefore, a combination of symptoms, pulmonary function tests, laboratory tests, radiographic procedures and biopsies is required for the accurate diagnosis, in particular to establish an occupational disease. The clinical course of the EAA can be acute/subacute or chronic and develop interstitial fibrosis with emphysema. In addition to the avoidance of allergen exposure, oral steroids are recommended as therapy. In chronic EAA immunosuppressive drugs can be added, if steroids are reduced.
