Abstrakt

In contrast to pulmonary arterial hypertension (PAH), pulmonary hypertension (PH) due to lung disease (PH-lung) and left heart disease (PH-LHD) is common. Recently, the term “atypical” PAH has been introduced for those PAH-patients who are older and offer more cardiac and pulmonary comorbidities than PAH-patients examined in clinical PAH-trials. PH-lung describes pre-capillary PH and comprises PH-COPD, PF-IPF, and PH-CPFE. By contrast, PH-LHD describes post-capillary PH and can complicate any left heart disorder. In both cases, the primary therapeutic goal consists in an improvement of the underlying condition. The use of PAHapproved therapies is not recommended; however, a minority of patients with clearly defined severe PH may be treated according to the recommendations for PAH, in the setting of PAH expert centers for individual patient care.
