Abstrakt
Transition is the transformation of the adolescent patient from the responsibility of the care-giving pediatrician into the hands of the adult medicine system. This transformation takes place during a difficult phase of live that means puberty with development of physical maturity, developing sexuality, vocational education, job finding and increasing autonomy. Transition is a dynamic process affecting the patient, the pediatrician, the adult physician and parents over a more or less prolonged time span, it is not only a transfer. Discontinuation of therapy with long term consequences due to different reasons is often and can be harmful for the patients. This article describes the actual situation of transition in the German health system.
