Health care transition – is it really a common task for pediatrics and pulmonology?

Abstrakt

Transition is defined as the purposeful, planned movement of adolescents and young adults with chronic physical and medical conditions from child-centered to adultoriented health-care systems, and usually is considered a “common” task for both parties. This paper argues that, although transition is a shared responsibility, the resulting tasks aren’t actually common, but very different. So far, there is little discussion about the tasks to be resolved by the adult health professionals. These may be conceptualized as: 1. approach to the (adolescent or young adult) patient in a developmentally appropriate way, 2. assessment of and looking after his or her residual needs, 3. coordinating and monitoring of the transfer, 4. facilitating access for new patients and their families. Such an understanding of “transition” shows that this process goes well beyond the mere transfer of a new patient.
