Treatment of pregnant women still remains a challenge for the surgeon and requires close interdisciplinary collaboration. Appendicitis, biliary tract diseases and ileus are among the most common illnesses in pregnancy requiring surgical intervention. Trauma is the leading cause of maternal and fetal deaths. Emergency operations can be performed at any time during pregnancy and can reduce late complications and mortality. Emergency operations should be postponed, if applicable, into the 2nd and 3rd trimester. Elective surgery should, however, be carried out after delivery, considering the risk to mother and child. Appendiceal surgery appears to be safer in open surgical techniques, compared to surgical interventions of the gallbladder, mostly performed by laparoscopy.
