Abstrakt

In bowel obstruction endoscopic procedures become more and more important. Stenoses of the gastric outlet and of the pyloroduodenal region can be treated by endoscopic recanalization in an effective and patient-safe way. In peritoneal carcinomatosis with recurrent vomiting, a PEG (percutaneous endoscopic gastrostomy) for decompression improves the quality of life significantly. In cases of acute obstruction of the large bowel, a primary endoscopic decompression can reduce the serious problems and risks of emergency surgery, which can be replaced by elective surgery (“bridge to surgery”). Decompression generally can be accomplished by endoscopic placement of a decompression tube or by primary stenting. Decompression tubes – till now underestimated – have clear advantages to stents such as better handling and less complication rates. Both methods, however – tube and stent – require a very experienced endoscopist who covers the entire field of endoscopic interventions. Same holds true for intestinal pseudoobstruction.
