Abstrakt

The technic of Acoustic Radiation Force Impulse (ARFI) is a non-invasive method to measure parenchymal elasticity and is incorporated in a conventional ultrasound system. Hepatic ARFI shear wave velocities show a good correlation with different degrees of hepatic fibrosis. Measurements were performed in the right hepatic lobe in an intercostal approach in fasting, supine patients with the convex ultrasound transducer. Clinical and laboratory data as well as the conventional B-mode ultrasound examination should be considered for interpretation of the ARFI elastography. Patients with cholangitis, obstructive cholestasis, right-heart insufficiency, elevated transaminases or hepatic failure show high, unreliable shear wave velocities. The major strengths of ARFI tissue quantification are the detection of significant fibrosis and the exclusion of cirrhosis. The classification in benign or malignant hepatic lesions by ARFI is not possible.
