Comprehensive overview of factors influencing performance in a 6-minute walk test in patients with COPD

Abstrakt
The aim of this study was to carry out a comprehensive overview of factors influencing performance in a six-minute walk test (6MWT) in patients with moderate to severe COPD. Patients and methods: The study included 82 patients with COPD (46 GOLD stage II and 36 GOLD stage III), 54 men and 28 women. 6MWTs were conducted according to the ATS guidelines, and walked distance (m) and the percent of predicted distance were associated with spirometric parameters (FVC, FEV1, FEV1/FVC ratio), transfer factor for carbon monoxide-DLco, transfer coefficient for carbon monoxide- Kco, the levels of dyspnea (Borg scale, Modified Medical Research Council Scale- MMRC), the levels of fatigue (Borg scale, Fatigue Assessment Scale-FAS), and nutrition status parameters (Body Mass Index – BMI and Fat-Free Mass Index – FFMI). Results: There was a significant difference between the two groups for 6MWT results – distance walked: GOLD II, 379 ± 82 m; GOLD III, 338 ± 77 m; p = 0.023; percent predicted: GOLD II, 73.3 ± 13.0%; GOLD III, 61.8 ± 12.6%; p = 0.000. 6MWT (% predicted) significantly correlated with FVC, FEV1, FEV1/FVC ratio, DLco, Kco, Borg dyspnea and fatigue scale changes from baseline, MMRC, FAS and BMI (p < 0.05 for all). Discriminant analysis (with exclusion of FVC and FEV1) depicted height, FEV1/FVC ratio, Kco, 6MWT (%), heart rate at the end of 6MWT and FAStotal score as the main predictor parameters for COPD severity stages having a very high agreement with COPD severity stages (GOLD II and III) based solely on FEV1 (kappa = 0.852; 95% CI). Conclusion: In patients with moderate to severe COPD 6MWT was significantly associated with lung function, gas transfer, level of dyspnea and fatigue, and nutrition status parameters. As a model show these tests a very high agreement with GOLD severity staging.
