Abstrakt

Mental disorders are of special importance in medical rehabilitation as they are frequent, often chronic and in many cases leading to disability. The question is, when special rehabilitative efforts are needed. This follows the paradigm of “critical incidents” (CI). Sociomedical CI are sick leave or work disability. Illness related CI are non-response, a negative turn in the illness course, or development of comorbidity. Biographical CI are a negative development after burdensome acute or structural life events, or living conditions which hinder treatment. In psychosomatic rehabilitation hospitals it must be clarified whether the present problem is an illness at all and of what kind, whether treatment options are still available, what impairment is given and whether this can be improved by training or changes in context, how the prognosis will be and whether quality of life can be improved. Sociomedical assessment is a stepwise process which must be embedded in the long-term ambulatory treatment.
