Inpatient and day care medical rehabilitation: course-setting in the health care and integration process of patients with chronic diseases

Abstrakt
In the care for patients with chronic illnesses it is of great importance to set the course for health-care and social integration under a bio-psycho-social orientation under a lifespan development perspective. Because of their special competences, rehabilitation hospitals play an important role in this process. They provide comprehensive diagnostic tools in reference to the International Classification of Functioning, Disability and Health (ICF). Characteristics of their work are a joint goal attainment process, shared decision making in the patient-doctor interaction and interdisciplinary team work, patient education programs, medical-vocational orientation, the consideration of sociomedical and occupational problems, the initiation of vocational rehabilitation, and the socio-medical appraisal. Self-directed physical activity, complex aftercare programs and stepwise vocational reintegration are initiated. Thereby, the course can be set not only for adequate long-term care with access to supporting systems, but also for activation, self-management and social participation.
