Abstrakt

An important prerequisite for the effective functioning of joint interdisciplinary rehabilitation processes, which include needs assessment, allocation to care as well as post-rehabilitation support, is the effective interdisciplinary collaboration between general practitioners, specialists in practice, rehabilitation centers and the pension insurance fund. However, the segmentation of the German health care system causes difficulties in collaboration mainly at the interfaces of the rehabilitation process. The main focus of this article lies on communication and collaboration between general practitioners (GP), rheumatologists, rehabilitation clinics and the German Pension Insurance Fund (Deutsche Rentenversicherung). The analysis focuses on the difficulties as well as potentials for optimization in the joint care for patients, with respect to a need-based access to rehabilitative care and interdisciplinary planning of the rehabilitative process, which transcends sectoral divides.
