Abstrakt

Liver resection represents the only potentially curative treatment option for patients with colorectal liver metastases (CRC-LM). Standard pretherapeutical staging includes contrast-enhanced CT-scan of thorax, abdomen and pelvis. In addition, an FDG-PET or PET-CT may be performed to exclude extrahepatic metastases more precisely. Prior to any treatment patients need to be discussed within an interdisciplinary tumor board including experienced hepatobiliary surgeons. To date, there is no clear evidence for pre- or postoperative chemotherapy in primary resectable CRC-LM. In contrast, in borderline/ potentially resectable CRC-LM the maximal tolerable chemotherapy should be applied to achieve secondary resectability. In patients with recurrent metastases, repeated surgical resection is again an option offering survival benefit. In patients with non-resectable CRC-LM, performance status, comorbidity, previous treatments, patient’s choice, toxicity and tumor related symptoms should be considered when deciding for first-line-treatment as well as further treatment strategies. Tumor directed therapy should be administered until progression or inacceptable side effects occur. However, intermittent therapy discontinuation and de-escalation strategies are possible.
