Abstrakt

Introduction: Crohn’s disease is a chronic inflammatory bowel disease, caused by a genetic predisposition in association with an intensive activation of the mucosal immune system. The clinical presentation differs by a different severity and different appearance. The therapy is based on a medical, surgical or a combination therapy. The therapy should always be coordinated interdisciplinary. Surgery in Crohn’s disease: The surgery is an essential part of therapy, even if it is not causal. The number of interventions has remained the same despite new drugs, laparoscopic surgery showed significant advantages over conventional surgery, but the procedure has no effect on the course of Crohn’s disease. Compared to conventional intestinal surgery for Crohn is associated with a higher morbidity. Surgery for stenosis: Strictures in Crohn’s disease may be inflammatory or fibrotic stenosis – where in particular the isolated short segment stenosis in the terminal ileum of inflammatory origin or fibrosis is readily accessible by surgical treatment. Surgery for intra-abdominal fistulas: Intra-abdominal fistulas are classified according to their course with special fistulas are an absolute indication for surgery, in combination with an abscess interventional drainage should be done first. Concomitant drug therapy should always be considered. Surgery for perianal fistulas: Perianal fistulas are common in Crohn’s disease and can’t be treated causally, because they have the original source in the rectum which takes part in Crohn’s disease. In order to preserve the sphincter intact the treatment can’t be causal. Surgical drainage with or without seton placement is often necessary for many years.
