Abstract

The management of sleep-related breathing disorders (SBAS) is currently undergoing fundamental changes due to many new scientific findings, but also frustrations with previous approaches. The often simplified approach to sleep-related breathing disorders in terms of severity classification, individual prognostic significance of the disease, therapy indication, and therapy monitoring, all based on a single parameter, the apnea-hypopnea index (AHI), does not do justice to the complexity of the clinical picture. A better understanding for the underlying pathophysiological mechanisms and their prognostic significance is essential for individualized therapeutic management. Knowledge of endotypes and phenotypes of sleep-related breathing disorders forms the basis for this. Endotypes outline the different underlying mechanisms of sleep-related breathing disorders and should be distinguished from clinical manifestations, the phenotypes.
