Abstract

Background: Outdoor workers have an increased risk for skin cancer. The German government therefore implemented a new occupational disease comprising squamous cell carcinoma and multiple actinic keratosis induced by exposure to occupational UVexposure. Objectives: We aimed to develop a skin cancer education for outdoor workers and investigate its effectiveness on knowledge on health risks and use of protective measures against solar ultraviolet radiation. Methods: A controlled cluster randomized intervention study with vocational school students in outdoor occupations was conducted including three follow-up surveys up to eight months after the intervention. We analysed two different intervention approaches (lecture and video). Results: We surveyed a total of 245 students. Age was identified as influencing factor on knowledge at baseline survey. Knowledge scores were improved significantly in the lecture intervention group, throughout all follow-ups. Improved knowledge had an effect on sunscreen use. The lecture had an effect on enhanced attitudes towards sun protection and improved sun protection behaviour. The most influential factor considering worksite prevention measures was found in provision of sun protection measures at the workplace. Conclusion: Our lecture-intervention improved knowledge on sun protection measures and health risks from excessive sun exposure. To ensure adequate sun protection behaviour at outdoor-workplaces, sun protection measures should be provided by the employer.
